The Doctor’s Guide to Weight Loss Surgery $3.48 Among the 55 percent of Americans who are overweight is a group of about five million who suffer from clinically severe obesity, also known as morbid obesity. This condition is a chronic disease associated with diabetes, high blood pressure, breathing problems and/or arthritis that can lead to further complications and death if not properly managed. Because of the high failure rate associated with dietary treatment, weight loss surgery, also known as "bariatric surgery," is the most effective method for obtaining sustained weight loss in severely obese people. However, misunderstanding and apprehension about its safety and effectiveness still abound. Louis J. Flancbaum, MD, Chief, Division of Bariatric Surgery at St. Luke’s-Roosevelt Hospital Center in New York City, a nationally recognized authority on the surgical treatment of obesity, has teamed up with former weight loss surgery patient, Erica Manfred, and freelance writer Deborah Biskin, to create the first comprehensive guide for the layperson, The Doctor’s Guide to Weight Loss Surgery: How to Make The Decision That Could Save Your Life. This personal and compelling in-depth exploration of the topic explains: · How the severity of obesity is graded and who is a candidate for weight loss surgery. · The types of operations used for obesity and the risks and benefits of each. · How weight loss surgery differs from liposuction and what kind of results can be expected. · How to choose a surgeon and a follow-up program. · Steps to improve the chances that your insurance company or third party payer will pay for the surgery. · The entire process of weight loss surgery, from pre-surgical evaluation, to the in-hospital experience, to what to watch out for in the months and years after surgery. The book also includes detailed illustrations of all the surgical procedures, nutritional guidance for post-ops, including gourmet high-protein recipes, online and print resources, and an extensive chapter of frequently asked questions. The Doctor’s Guide to Weight Loss Surgery explodes a number of myths about this controversial procedure: Myth: Weight loss surgery and stomach stapling are the same thing. Reality: Stomach stapling refers to an older, simpler and less effective type of surgery, called gastroplasty, which often resulted in unhealthy eating patterns and staple line disruptions. The newer operation, Roux-en-Y gastric bypass (RYGB), partitions the stomach and rearranges the intestine. It encourages healthier eating, has a higher rate of permanent weight loss and rarely causes staples to pop. Myth: Weight loss surgery is extremely dangerous. Reality: Morbid obesity is extremely dangerous. Obese people are at higher risk for any major surgical procedure. Despite their higher risk, however, the incidence of death related to weight loss surgery is 1%–similar to open heart surgery. Weight loss surgery is no more dangerous for a morbidly obese person than any other … |

Goin’ to Kansas City [2003] $16.98 Jay McShann is one of Kansas City’s great treasures, and this disc brings him back to his hometown to record for the first time in almost 50 years, although he’s lived there for many decades. Made piecemeal over a few years (although it hangs together so well that you wouldn’t know it), these are very definitely the same Kansas City blues that are at the root of music from Charlie Parker and Count Basie. A marvelous, effortless pianist, he seems happiest on material with a strong connection to the blues, like “Kansas City,” which comes in two versions, or his very rooted version of “Ain’t Nobody’s Business.” And when he gets to the completely solo “The Fish Fry Boogie,” age slips completely from his fingers, with some spirited stride playing and a strong left hand under the dexterous right. The excellent Duke Robillard lends strong support on several tracks on acoustic and electric guitars, Maria Muldaur duets on “Confessin’ The Blues,” and the great Johnnie Johnson sits in for “Some Kinda Crazy” and one of the versions of “Kansas City,” which really catches fire. The whole album is an absolute joy, and covers a lot of ground, including the long interview with McShann that forms the long bonus track — and includes more of his playing. It’s great to know that they can still make blues like this. ~ Chris Nickson, All Music GuidePerformers: Johnnie Johnson – Piano, Vocals; Jay McShann – Piano, Vocals; Maria Muldaur – Vocals; Duke Robillard – Guitar (Acoustic), Guitar (Electric) |

The Kansas City Sessions $9.71 This is a CD overflowing with classic performances. The great Lester Young is heard with the 1938 version of the Kansas City Six, a group also including trumpeter Buck Clayton (in prime form), Eddie Durham on electric guitar (where he preceded Charlie Christian) and trombone, the rhythm guitar of Freddie Green, bassist Walter Page, and drummer Jo Jones. The four selections (all of which are joined by an alternate take apiece) are most notable for Young’s switching to clarinet on some of the pieces. His clarinet solo on “I Want a Little Girl” sounds eerily like Paul Desmond’s alto of 15 years later. These classic cool jazz performances have delightful interplay between the two horns. The second part of the reissue features the 1944 Kansas City Six in which Young (sticking to tenor) and trumpeter Bill Coleman are joined by a three-piece rhythm section and trombonist Dickie Wells. Wells, who takes some very colorful and nearly riotous solos, rarely sounded better, and the four selections are highlighted by three equally rewarding versions of “I Got Rhythm” and two of “Three Little Words.” The CD concludes with the four titles by 1938’s Kansas City Five, which was essentially the early Kansas City Six without Young. Clayton is once again in top form, and Durham’s guitar solos were among the first worthwhile examples of the electric guitar on record. This gem is highly recommended for all jazz collections. ~ Scott Yanow, All Music GuidePerformers: Joe Bushkin – Piano; Buck Clayton – Trumpet; Bill Coleman – Trumpet; Eddie Durham – Guitar (Electric), Trombone; Freddie Green – Guitar, Vocals; Jo Jones – Drums; Walter Page – Bass; John Simmons – Bass; Dicky Wells – Trombone; Lester Young – Clarinet, Sax (Tenor) |

Kansas City [Original Soundtrack] $14.98 For Robert Altman’s Kansas City film, since the story was centered in 1934 Kansas City, Altman wanted to have younger musicians depict top jazz artists of the era playing at one of the legendary jam sessions. He recruited many of today’s top modernists and, although they used arrangements based on older recordings, they did not have to necessarily improvise in the style of the time. Actually, it is surprising how close the musicians often come, recapturing not just the music of the period but the adventurous spirit of such immortals as Coleman Hawkins, Ben Webster, and Lester Young. A dozen songs from the film are on this very enjoyable and unique CD, which features such players as trumpeter Nicholas Payton, clarinetist Don Byron, guitarists Russell Malone and Mark Whitfield, pianists Geri Allen and Cyrus Chestnut, altoists Jesse Davis and David “Fathead” Newman, and four of today’s great tenors: James Carter, Craig Handy, David Murray, and Joshua Redman. In addition, Kevin Mahogany sings “I Left My Baby.” Although there are some audience shouts on a couple of the pieces, this is one soundtrack album that very much stands up on its own. ~ Scott Yanow, All Music GuidePerformers: Olu Dara – Cornet; Geri Allen – Piano; Don Byron – Clarinet, Sax (Baritone); James Carter – Sax (Tenor); Ron Carter – Bass; Tyrone Clark – Bass; Jesse Davis – Sax (Alto); Curtis Fowlkes – Trombone; Clark Gayton – Trombone; Craig Handy – Sax (Tenor); Victor Lewis – Drums; Kevin Mahogany – Vocals; Russell Malone – Guitar; Christian McBride - |
